29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b).    a) Michelle's Home Improvements, LLC dba Triple Star Home Improvements on or about May 23, 2012, at the address of 335 Wolf Creek Ct. in Northwood, OH: The employer did not ensure employees were protected from falls of up to approximately 18 feet while engaged in residential roofing activities on the two story residential structure.    The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA Inspection Number 311603872, citation number 1,  item number 1 and was affirmed as a final order on July 8, 2008, with respect to a workplace located at 28362 Fostoria Rd. in Millbury, Ohio.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
