29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.       a) At the facility, 315 Beach 9th Street, Far Rockaway, N.Y - Dish Wash Room - On a routine basis, maintenance employee(s) change out five (5) gallon containers of products including Rapid Dry Rinse / Rinse Aid (pH:3.0; Corrosive), Sani #18 (pH: 12.2; Caustic), and Ron - X Super Machine Detergent ( pH: 12 Causic). These products contain hazardous chemicals including, but not limited to,sodium hypochlorite, potassium hydroxide, sodium metasilicate. The employer did not perform a written hazard assessment on or about 06/06/12 and 11/20/12.    b) At the facility, 315 Beach 9th  Street, Far Rockaway, N.Y - Boiler Room - On a routine basis maintenance staff enters into the boiler room identified by a sign as containing asbestos. Employee(s) turn valves on / off, and perform regular maintenance activities including, but not limited to, maintenance on the pipes containing ACM or PACM as required. the employee(s) are exposed to the hazards associated asbestos containing material (ACM). The employer did not perform a written hazard assessment on or  about 06/06/12 and 11/20/12.    c) At the facility, 315 Beach 9th Street, Far Rockaway, N.Y. - Throughout the facility - On a routine basis,maintenance employees used products to include spray enamels to touch up walls, doors, and other damamged areas within the facility. The enamels contain hazardous chemicals including, but not limited to, propane, butane, solvent naphtha, and benzene. Employees are exposed to the hazards associated with these chemicals. The employer did not perform a written hazard assessment on or about 06/06/12 and 11/20/12.      ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.1

On a routine basis maintenance staff enters into the boiler room identified by a sign as containing asbestos. Employee(s) turn valves on / off, and perform regular maintenance activities including, but not limited to, maintenance on the pipes containing ACM or PACM as required. In the room there are some pipes that are insulated with fiberglass and others with ACM or PACM. The employees are not able to distunguish between the two insulations as the employer did not affix warning lables to the material contianing ACM and / or PACM on or about 11/20/12.    ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.1
