29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about June 7, 2012, and at times prior thereto, in two instances the employer allowed employees to operate various machines without proper machine guarding:    Instance 1: At the CNC machines areas an employee was observed operating a CNC machine, manufactured by Leadwell, Model T-6, which did not have an interlock device provided on the door.    Instance 2: At area #5 an employee was observed operating an engine lathe machine, manufactured by Clausing-Metosa, without using a chuck guard.     Employees were exposed to injury caused by the unguarded chucks while in operation.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      On or about June 7, 2012, and at times prior thereto, in two instances in the glue and hot wax areas CSHO observed commercial grade electrical relocatable-power-taps (RPT) were being used:    Instance 1: At the glue area two heating lamps were found plugged into an RPT    Instance 2: At the hot wax area a heating pot was found plugged into an RPT.      That was not in accordance with instructions of the listed or labeled equipment.  Employees were exposed to fire hazards.

29 CFR 1910.1200(h)(1):     Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      On or about June 7, 2012, and at times prior thereto, at the facility employees were not provided with effective information and training on hazardous chemicals such as, Polyamide Resin, in their work area at time of their initial assignment and whenever a new hazard was introduced into their work area.  Employees were exposed to hazardous chemicals.
