29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about June 7, 2012, and at times prior thereto, in two instances the employer allowed employees to operate various machines without proper machine guarding:    Instance 1: At the CNC machines areas an employee was observed operating a CNC machine, manufactured by Leadwell, Model T-6, which did not have an interlock device provided on the door.    Instance 2: At area #5 an employee was observed operating an engine lathe machine, manufactured by Clausing-Metosa, without using a chuck guard.     Employees were exposed to injury caused by the unguarded chucks while in operation.

That was not in accordance with instructions of the listed or labeled equipment.  Employees were exposed to fire hazards.
