29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    (a) Construction Site, Station Interior - On or about June 8, 2012 an employee working on a mobile supported scaffold was drilling into the exterior wall and the working level was not fully decked.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) Construction Site, Station Interior - On or about June 8, 2012 an employee drilling into an exterior wall from a mobile supported scaffold was  not protected from falling approximately 13 feet to grade.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold is used in a stationary manner.    (a) Construction Site, Station Interior - On or about June 8, 2012 an employee was working on a mobile supported scaffold and the scaffold wheels were not locked in place.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
