29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or an equivalent dose of fifty percent:      a) Iridite/Packing:  The employee, who was involved in removing moisture from the plated metal parts with compressed air, was exposed to noise that was above the Action Level of 85 dBA on 9/12/2012.  The continuous noise level was 90.7 dBA which was equivalent dose percent of 104 % of the allowable 8-hour Time Weighted Average Action Level of 85 dBA. The total sampling time was 460 minutes and zero exposure was assumed for unsampled period of time.

The employee, who was involved in removing moisture and fine particles from plated metal parts with compressed air, was exposed to sound level that was 90.7 dBA as an eight-hour time weighted average, on or about 9/12/2012.

29 CFR 1910.176(c): Storage areas shall be kept free from accumulation of materials that constitute hazards from tripping, fire, explosion, or pest harborage. Vegetation control will be exercised when necessary.    a) General Storage Area:  3 55-gallon drums of nitric acid and 350 pounds of sodium hydroxide were stored together, which can cause a vigorous reaction if the two chemicals came into contact with each other, on or about 6/8/2012.
