29 CFR 1926.453(b)(2)(v):  A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    1100 Laurel St., Baton Rouge, LA:  On or about June 1, 2012, the employer did not ensure that employee(s) wore a safety harness when working from the basket of an aerial lift.  Employee(s) were installing rivets and hip flashing to a roof, working at approximately 20 ft. high.  Employee(s) were exposed to a fall hazard.

29 CFR 1926.501(b)(11):  "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    1100 Laurel St., Baton Rouge, LA:  On or about June 1, 2012, the employer did not ensure that employee(s) working on a 10/12 pitch roof wore adequate fall protection.  Employee(s) were installing rivets and hip flushing (corner metal) on a roof with a ground to eave height of approximately 20 feet.  Employee(s) were exposed to a fall hazard.
