29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:      (a)  On or about 05/22/12 - 11104 Dr. Edward Hilliard Drive, Tuscaloosa, AL, Building #2, an employee was exposed to a 24 feet fall hazard when climbing scaffold components to access the scaffold.

29 CFR 1926.451(g)(4)(xv):     Crossbracing used in place of a mid-rail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top-rail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a top-rail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:      (a)  On or about 05/22/12 - 11104 Dr. Edward Hilliard Drive, Tuscaloosa, AL, Building #2, a mid-rail was not provided where the crossing point of crossbracing was more than 30 inches above the work platform.

29 CFR 1926.451(h)(2)(ii):     A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:  Note:  This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toeboards.    (a)  On or about 05/22/12 - 11104 Dr. Edward Hilliard Drive, Tuscaloosa, AL, Building #2, toeboards were not installed along the edge of the scaffolding platforms where materials such as, but not limited to bricks, mud pans, buckets and other tools and materials were stored.
