29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: (a) On or about 05/22/12 - 11104 Dr. Edward Hilliard Drive, Tuscaloosa, AL, Building #2, an employee was exposed to a 24 feet fall hazard when climbing scaffold components to access the scaffold.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a mid-rail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top-rail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a top-rail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart: (a) On or about 05/22/12 - 11104 Dr. Edward Hilliard Drive, Tuscaloosa, AL, Building #2, a mid-rail was not provided where the crossing point of crossbracing was more than 30 inches above the work platform.
