29 CFR 1926.453(b)(2)(v): The employer does not always ensure that employees wear fall restraint (full body harness) with a lanyard attached to the boom or basket when working from an aerial lift. On or about May 21, 2012, the employer failed to ensure that employee(s) wore a fall restraint when working in the basket of the extendable boom aerial lift. The employee was observed working at a height of approximately 20 feet.
