29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:    a)  Employees were not provided with a safe means of egress when working in a trench with a depth of approximately 10.7 feet.    No abatement certification or documentation is required for this item.

29 CFR 1926.652(g)(2):  Excavations of earth material to a level greater than 2 feet (.61m) below the bottom of a shield was permitted:    a)  Employer permitted its employees to enter the trench and perform work when the earth material was excavated approximately 5 feet below the bottom of the support system.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
