The employer is failing to protect employees from potential emergency egress hazards associated with not marking emergency exits clearly with a sign that reads Exit. This was most evident during the inspection of the facility located at 3900 Industrial, Lincoln, NE 68504 where the employer has employees working on the production floor and the doors that the employer has designated to be emergency exits are not clearly marked as such. Instances include but are not limited to: i. The exit door located next to the Hyd-Mech Horizontal Band Saw is not marked and when all of the overhead doors are pulled closed it is the only exit door on that side of the building.

The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable: The employer failed to develop a written respiratory program mandated for required respirator use.
