The area also was infested with mice.

There was evidence of mice droppings in the break room in the facility.  During the inspection it was observed that mice were running through the restroom and hallways.

Nor are they requiring/providing appropriate personal protective equipment for employees adjacent to the welding operations in the facility.

The employer is failing to protect the employees from struck-by hazards in the back storage room/mezzanine area.  Materials are being stored on the roof of this room.  The roof area has not been rated as to the safe weight which can be placed upon it.  This rating has not been posted to alert workers to the safe weight it will support

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   The employer is failing to provide Hazard Communication training to employees either during their initial assignment or whenever a new physical or health hazard that the employee has not previously been trained for has been introduced into their work area.  Some of the employees in the facility had not received training on the hazards of the chemicals/materials in the facility in which they could be potentially exposed.
