29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. On or about June 15, 2012, an owner was operating a table saw cutting the steel frames off of soda machine condenser coils and the point of operation of the table saw was not guarded, exposing the owner to laceration and amputation hazards. b. On or about June 15, 2012, an owner was operating the baler machine to compress soda machine condenser coils and were not protected from the point of operation of the baler ram, in that there was no top slide gate guard covering the above portion of the ram piston during the operating cycle, exposing the owner to amputation and crushing hazards. c. On or about June 15, 2012, the owners was operating an hydraulic shear to separate the copper parts of the vending machine valves from the steel parts and the point of operation was not guarded, exposing the owner to amputation hazards.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface: a. On or about June 15, 2012, employees working in the junk table area were exposed to electrical and fire hazards because the faceplate for the light switch was broken in half, exposing the internal wires.
