29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a)  On or about June 21, 2012, employees were exposed to shock and burn hazards from a cord they were using that had cut and frayed insulation on a construction site located at 16173 Old Forest Pt. in Monument, Colorado.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
