29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection. This violation most recently occurred on June 22, 2012 at a job site located at 3535 Barnes Bridge Rd, Dallas, Texas, an employee who was securing siding, worked from the second tier of a scaffold and was not protected from falling more than 12-feet 2-inches, respectively, to the lower level.
