29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units:    (a) On the west side of the project near the electrical service entrance pole on or about June 20, 2012, the unstable concrete masonry units and 2 by 6 pieces of lumber were used to support the legs of a tubular welded frame scaffold.  The scaffold platform was 6 feet, 8 inches high.  Employees were on the platform to install the electrical service entrance.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed:    (a) On the west side of the project near the electrical service entrance pole on or about June 20, 2012, the step ladder was used in the folded up position to access a scaffold.  The scaffold platform was 6 feet, 8 inches high.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    (a) On the west side of the project near the electrical service entrance pole on or about June 20, 2012, empty cement bags, pieces of rebar, and tile debris were not kept clear from the area in front of the electrical outlets.  Employees plugged extension cords into the outlets.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a) On the first level of the building on or about June 20, 2012, eye protection was not worn while plastering the wall with Portland cement with a trowel above the head level.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:    (a) On the west side of the project near the electrical service entrance pole on or about June 20, 2012, there was no assured equipment grounding conductor program or ground-fault circuit-interrupters to protect employees that used a 110 VAC Hilti hammer drill and a 110 VAC DeWalt drill to install the electrical service entrance box and conduit.  The tools were powered through an extension cord.
