29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a)  Roof Check, Inc. at 1878 Tiverton Ave, Broomfield, CO 80023:  On June 23, 2012, the exposing employer did not ensure that employees were protected from injury, in that one employee installing vent covers on the 2nd level roof, roof pitch of 5-in-12, on a two-story residential home was not utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 22 feet.    (b)  Roof Check, Inc. at 1878 Tiverton Ave, Broomfield, CO 80023:  On June 23, 2012, the exposing employer did not ensure that employees were protected from injury, in that one employee working on east side of the 1st level roof, roof pitch of 4-in-12, was not utilizing a fall protection system. This condition exposed the employee to a fall hazard of approximately 10 feet.    Roof Check, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 313420176, Citation Number 2, Item 2, issued on September 15, 2010, and became Final Order on November 1, 2010, with respect to a workplace located at 15317 E. 101st Pl., Commerce City, CO 80022.
