29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      (a) On or about April 25, 2012, at a job site located at 601 W. 103rd Street in Kansas City, MO, employees were exposed to fall hazards while transitioning from an extension ladder to an upper landing surface due to the ladder's side rails not extending the required 3 feet above the upper surface.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a safety monitoring system:      (a) On or about April 25, 2012, at a job site located at 601 W. 103rd Street in Kansas City, MO, employees were exposed to fall hazards of greater than 6 feet while performing roofing work on a large commercial flat roof, over 50 feet wide in its least dimension, without any type of an acceptable fall protection system being in place or in use at the time of the inspection.      BIELAK ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310939673, CITATION NUMBER 1, ITEM NUMBER 4, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 6, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 6700 NORTH OAK TRAFFICWAY IN GLADSTONE, MO.
