On or about June 12, 2012 and at times prior thereto, employees were using a circular saw and were not provided with eye protection.  They were exposed to the hazard of having saw dust and debris fall into their eyes.

29 CFR 1926.304(d):   Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR EQUIPMENT, INCLUDING SERIAL NUMBER, MODEL, ETC.)(KIND OF WORK BEING DONE)(DESCRIBE WHICH GUARD IS MISSING)    On or about June 12, 2012 and at times prior thereto, employees were using the circular saw with the blade guard tied in the upper position, preventing it from covering the blade when the saw was not in use.  Employees were exposed to cuts.

29 CFR 1926.501(b)(13):   Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    On or about June 12, 2012 and at times prior thereto, employees were working at a height over 10 feet and were not provided fall protection.  They were exposed to a fall of over ten feet.
