29 CFR 1926.21(b)(3): Employees were not instructed in the avoidance of unsafe conditions: a) On or about May 18, 2012, the employer did not ensure that the gas pipeline located just below the floor of an excavation was properly protected before employees ran over it with a large tiller. On May 18, 2012, a large gas leak was created when the tiller tore off the top of the gas line.
