OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fall hazards: (a) On or about 06/09/2012 - Weld Department, Line 1, an employee working from a Sky Jack scissor lift was pulling a deenergized conductor with a KELLUMS cable grip and fell 16 feet to the ground. The lift was struck by the conductor that was being pulled by a company truck. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the instructions for the installation of the KELLUMS cable grip, as stated in the HUBBELL Wiring-KELLUMS Catalog, 2011.
