Warehouse:  On or about June 28, 2012 and times prior thereto, a synthetic sling available for use had cuts/tears past the warning threads.  This exposed employees to struck-by hazards from the slinging breaking during use.    Saxman Yard:  On or about June 28, 2012 and times prior thereto, a synthetic sling available connected to a crate had cuts/tears past the warning threads.  This exposed employees to struck-by hazards from the slinging breaking during use.

Saxman Maintenance Yard:  On or about June 28, 2012 and times prior thereto, the chains used in the shop did not have quarterly inspections completed.  This exposed employees to struck-by hazards if the chain was to break.

29 CFR 1917.112(b)(1):     Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet.    Saxman break room.  On or about June 28, 2012 and times prior thereto, employees were exposed to a 88 inch fall from the top of floor were they stored supplies.  This exposed employees to a fall hazard causing broken bones and contusions.

Dockside warehouse:  On or about June 28, 2012 and times prior thereto., the pedestal crane used to load tug boats had not been examined by a certified inspector.  This exposed employees to struck-by hazards from mechanical discrepencies found on the crane.
