29 CFR 1917.151(g)(1):     Safety guards for abrasive wheels were not adjustable or did not have an adjustable tongue or piece at the top of the opening where the operator must stand in front of the safety guard opening.      AMAK Warehouse (Maintenance area):  On or about 28 June 2012 and times prior thereto, the Baldor F1075 Grinder, serial #G7-171-30, was missing the tongue guard.  The equipment was plugged into the 120 volt, electrical outlet.  This exposed employees to struck by hazards.

29 CFR 1917.151(g)(4):     Work rests on fixed grinding machines were not adjusted closely to the wheel with a maximum opening of 1/8-inch (3.18 mm) and securely clamped.    AMAK Warehouse (Maintenance area):  On or about 28 June 2012 and times prior thereto, the Baldor F1075 Grinder, serial # G7-171-30, work rest was unsecured.  This exposed employees to struck by and caught in hazards.

29 CFR 1917.151(h)(1):     Rotating parts such as gears and pulleys located 7 feet (2.13 m) or less above working surfaces were not guarded to prevent employee contact with moving parts.    Saxman Yard (welding Shop area):  On or about 28 June 2012 and times prior thereto, the locally manufactured grinder adjacent to the Miller welding unit, had exposed belts and pulleys.  The grinder was plugged into a 120 volt electrical outlet.  This exposed employees to pinch hazards.

29 CFR 1917.152(e)(2)(ii):     Arc welding or cutting cable was used when it was not free from repair or splice for 10 feet (3 m) from the electrode holder.    Saxman Yard (Welding shop area):  On or about 28 June 2012 and times prior thereto, the Miller 250 Welding unit, Serial # KF825154, Stock # 903056, was connected to a stinger with a burnt hole that exposed wires of the lead.  The exposed wires were 3 inches from the stinger and were improperly repaired with black electrical tape.  This condition exposed employees to electrical shock hazards.

29 CFR 1917.152(c)(3):     Fire extinguishing equipment suitable for the location was not immediately available or maintained in readiness for use at all times during the performance of hot work.      On or about 28 June 2012 and times prior thereto, the fire extinguishers at SaxmanYard were not inspected on a monthly basis.  Stick welding operations were being conducted in the welding shop building.  This exposed employees to smoke inhalation hazards in combatting fires.
