29 CFR 1917.152(e)(2)(ii):     Arc welding or cutting cable was used when it was not free from repair or splice for 10 feet (3 m) from the electrode holder.    Saxman Yard (Welding shop area):  On or about 28 June 2012 and times prior thereto, the Miller 250 Welding unit, Serial # KF825154, Stock # 903056, was connected to a stinger with a burnt hole that exposed wires of the lead.  The exposed wires were 3 inches from the stinger and were improperly repaired with black electrical tape.  This condition exposed employees to electrical shock hazards.
