29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a)  AMAK Warehouse Area:  On or about 28 June 2012 and times prior thereto, the 120 volt, Milwaukee vacuum cleaner's electrical plug was missing the ground pin.  This exposed employees to electrical shock hazards.      b)  AMAK Warehouse Area:  On or about 28 June 2012 and times prior thereto, the 120 volt, Paint shaker cord had exposed wires. This exposed employees to electrical shock hazards.      c)  Saxman Yard (Welding shop area):  On or about 28 June 2012 and times prior thereto, the 120 volt, Makita circular hand saw, 7 1/4" blade, Model # 5277NB (Ser # 00541377) had a damaged cord. The electrical cord was taped with black electrical tape.  This exposed employees to electrical shock hazards.
