29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At the south side of building 12, on July 5, 2012, two employees were applying caulk to the apartment window frames from the lower roof. The employees were exposed to falls of approximately 16.8 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the south section of building 12, on July 5, 2012, the portable metal ladder used to gain access to lower level roof extended approxiimately 1.5 above the landing surface. This condition exposed employees to slips and falls.
