29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.     (a) 200 Central Avenue, window ledge. Employee was painting on a 4 feet by 6 feet ledge above a window without guardrails or personal fall arrest system 10 1/2 feet from lower level. Observed on or about 9 July 2012.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.    (a) 200 Central Avenue, White Plains New York. Employee used a ladder that did not extend at least 3 feet above the upper landing surface of a window ledge 10 1/2 feet above a lower level. Observed on or about 9 July 2012.
