29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Production: Cold headers were not safeguarded in accordance with American National Standard Institute (ANSI) B11.7, "Cold Headers and Cold Formers - Safety Requirements for Construction, Care, and Use," in that: A. The interlock to the guard was bypassed while in run mode on the Ayase Seiki Co., Type ASOOHTR, Number 12081 header. B. The following equipment was operated without the ram guard interlocked: 1. Asahi Okuma, Machine T-48-1187 2. National Four Die 3. Nakashimada N E W H-20A 7906252 C. The Asahi Okuma, Machine T-48-1187 was operated without the hood guard interlocked. D. The hood guard was not secured on the National Four Die header. E. The National Four Die header was provided with a single jog button for setup operation. F. The company did not provide spring loaded turnover bars which would prevent them from being left in the flywheel. G. The company did not have established written die setting procedures for the header setup. H. The employer did not provide die blocks for use during setup.
