On or about Monday, July 2, 2012 at 41-15 45th Street, Sunnyside, NY 11104    The disconnects of an electrical panel box located in the basement of the building used by employees to power two (2) point suspension scaffold motors and additional equipment was observed missing labels indicating disconnect functions exposing employees to electrical hazards.

29 CFR 1926.451(d)(16):     The suspension scaffold power-operated hoist or manually operated hoist did not have, in addition to the normal operating brake, a braking device or locking pawl which engages automatically when a hoist makes either of the followin uncontrolled movements: an instantaneous change in momentum or an accelerated overspeed.    A.) On or about Monday, July 2, 2012 at 41-15 45th Street, Sunnyside, NY 11104    A suspension scaffold erected approximately sixty (60) feet above the lower level used by employees during a brick pointing operation did not have an additional functional braking device in place that engaged automatically following an uncontrolled movement exposing employees to fall hazards.

A.) On or about Monday, July 2, 2012 at 41-15 45th Street, Sunnyside, NY 11104    The vertical lifelines of a two point suspension scaffold erected approximately 60 feet above the lower level were observed attached to the same point of anchorage as the scaffold's suspension lines exposing employees to fall hazards.
