On or about Monday, July 2, 2012 at 41-15 45th Street, Sunnyside, NY 11104 The disconnects of an electrical panel box located in the basement of the building used by employees to power two (2) point suspension scaffold motors and additional equipment was observed missing labels indicating disconnect functions exposing employees to electrical hazards.

On or about Monday, July 2, 2012 at 41-15 45th Street, Sunnyside, NY 11104 A suspension scaffold erected approximately sixty (60) feet above the lower level used by employees during a brick pointing operation did not have an additional functional braking device in place that engaged automatically following an uncontrolled movement exposing employees to fall hazards.
