29 CFR 1910.176(a):     Permanent aisles or passageways were not appropriately marked:    a) On or about June 26, 2012 , the main aisles and passageways that ran through the Puma Department which led to the  lunchroom and restroom facility areas were not permanently marked to identify aisles turns, and marked to control foot traffic and material handling equipment such as fork trucks which occupied the same aisles and passageways at the same time.

29 CFR 1910.178(l)(4)(ii)(B):     Refresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near-miss incident    a) On or about June 26, 2012, and after a recorded accident that involved a fork truck operator striking an employee with the counter weight section of Hyster reach fork truck. The operator was not retrained in the knowledge and skills needed to operate powered industrial trucks safely and to maintain a clear safe distance from any employee(s) walking and or working near fork truck in operation.
