29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  On or about June 21, 2012, employees were exposed to fall hazards while performing residential construction activities to include the following:    a.  Employee was exposed to fall hazards of approximately 12.4 feet to the outside ground and 9.9 feet to the inside  concrete, while performing insulation wrapping operations on the garage window of the home.        b.  Employee was exposed to fall hazards of approximately   11 feet to the outside ground and 14.7 feet to the inside of the wooden flooring while performing insulation wrapping operations to the dormers of the home.    c.  Employee was exposed to fall hazards of approximately 12.4 to the outside ground while performing insulation wrapping operations to second story window area.        SEI Group was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number#315742320 citation 2 Item 1 and was affirmed as a final order on 6/12/2012.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) The employer failed to ensure that each employee potentially exposed to fall hazards was properly trained to recognize and minimize fall hazards while performing insulation wrapping operations.    SEI Group was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1) which was contained in OSHA inspection number#312775166 citation 1 Item 2 and was affirmed as a final order on 1/18/2010.
