29 CFR 1926.21(b)(2):   The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) 4 Whitehill Dr. West Hartford, CT. - On or about 7-12-12 - Employee was observed using a 2 inch by 10 inch wood plank as a ramp to dump roofing debris into the side of a trailer with a wheelbarrow.  Side of wheeled trailer was 6 and 1/2 feet above the ground.  Ramp was not at least 18 inches wide.

The grounding pin on the cord was missing and no GFCI was in use on the site.

The saw was being used with ungrounded extension cord and no GFCI.
