Andersen Air Force Base nearby the AAFES Exchange and the base security office, Yigo, Guam. On or about July 16, 2012, and at times prior thereto, the employer failed to provide protection for employees by placing and keeping excavated materials at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  Excavated materials were between 1/2 inch to 1 foot away from the excavation around the edge of the trench exposing workers to serious injuries from falling or rolling debris.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:    Andersen Air Force Base nearby the AAFES Exchange and the base security office, Yigo, Guam. On or about July 16, 2012, and at times prior thereto, the employer failed to provide protection for employees working in an unguarded trench seven feet (7') in depth.  Adequate retaining, protective device, or sloping to prevent cave-ins was not provided for type B soil classified as texturally silty gravelly sand, with structurally granular cohesiveness.
