29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a)PetraBee Construction at 3617 Pinewood Ct., Johnstown, CO 80534: On July 16, 2012 the employer did not ensure the working platform on the fabricated frame scaffold was fully planked in that the scaffold was 3 feet wide and two levels high and 18 inches were planked on each level. This condition exposed the employee to a fall hazard of up to approximately 14 feet.

The working surface of the scaffold was 14 feet high and the scaffold was 3 feet wide. This condition exposed the employee to a fall of up to approximately 14 feet.
