29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:      (a)PetraBee Construction at 3617 Pinewood Ct., Johnstown, CO 80534:  On July 16, 2012 the employer did not ensure the working platform on the fabricated frame scaffold was fully planked in that the scaffold was 3 feet wide and two levels high and 18 inches were planked on each level.  This condition exposed the employee to a fall hazard of up to approximately 14 feet.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and be repeated vertically at locations of horizontal members every 20 feet (6.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less:      (a)PetraBee Construction at 3617 Pinewood Ct., Johnstown, CO 80534:  On July 16, 2012 the employer did not ensure the fabricated frame scaffold was tied into the building at a 4:1 height.  The working surface of the scaffold was 14 feet high and the scaffold was 3 feet wide.  This condition exposed the employee to a fall of up to approximately 14 feet.

29 CFR 1926.452(c)(2):  Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secured vertical members together laterally:    (a)PetraBee Construction at 3617 Pinewood Ct., Johnstown, CO 80534:  On July 16, 2012 the employer did not ensure all brace connections were secured on the fabricated frame scaffold in use in that there were no horizontal or cross bracing on the second stage of the scaffold.  This condition exposed the employees to a fall of up to approximately 14 feet.

29 CFR 1926.451(e)(1):  Scaffold platforms that were more than two feet (0.6m) above or below a point of access were not provided with a means of access:    (a)PetraBee Construction at 3617 Pinewood Ct., Johnstown, CO 80534:  On July 16, 2012 the employee did not ensure a ladder was provided to allow for safe access and egress to the fabricated frame scaffold.  Without the ladder the employee climbed the bracing.  This condition exposed the employee to a fall hazard of approximately 14 feet.

This condition exposed the employee to a fall of approximate 14 feet.
