29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: A. In the RMA area, relocatable power taps were used against listed instructions and were daisy chained together to power equipment including a Slaughter Model 1101 Dielectric Breakdown tester and GW Instek GPR-11H300 Laboratory DC Power Supply, and metal-encased magnifying inspection lamp. B. In the following locations, wall-mount listed electrical boxes with knockouts were used against listed instructions as pendant power drops: 1. RMA Area: Box was lying on the desk, unsecured, and powered equipment such as a computer monitor, printer, computer and metal-encased floor fan. 2. RMA Area: At the back of the area, behind the blue pegboard wall, the box was powering a relocatable power tap which in turn powered a Keurig coffee maker. 3. IDI Stock Room: To the right of the work bench, the box was powering the Mettler Toledo scales.

B. 277/480V "Panel 2" was blocked by the placement of the Kodiak battery charger. C. 240V "Panel 3" was also blocked by the placement of the Kodiak battery charger. D. 480V "Panel 4" was blocked by the Drexel fork truck, which was parked 15" from the face of the panel.
