29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    On or about July 18, 2012, and at times prior thereto, at its excavation Santa Rosa worksite in Santa Rita, Guam, the controlling employer, South Gulf Inc., failed to ensure that its sub-contractor, Joda International, protected a shaft/excavation 6 feet and 8 inches in depth with a cover, fence, barrier, or guardrail system; exposing employees to serious injuries from falls into the excavation.  The shaft opening was also not readily visible and obscured by plant growth and soil mounds.
