29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    On or about July 18, 2012 and at times prior thereto, at its excavation worksite in Guam, the employer failed to protect a shaft/excavation measuring 6 feet and 8 inches in depth with a cover, fence, barrier, or guardrail system exposing employees to serious injuries from falls into the excavation.  The shaft opening was also not readily visible and obscured by plant growth and soil mounds.
