29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables were used:    a)  10 N. Maple Ave., Marlton:  An employee was exposed to electrical hazards while using a worn and frayed extension cord to power a Makita 7  inch hand operated circular saw used to cut 2x6 dimensional lumber.  Observed on or about 07/12/2012.     NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  10 N. Maple Ave., Marlton:  Employees were exposed to fall hazards while installing trusses and and framing a steep sloped (8 on 12 pitch) roof with no fall protection and were exposed to a fall hazard of greater than 21 feet.  Observed on or about 07/12/2012.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    a)  10 N. Maple Ave., Marlton:  Employees were exposed to fall hazards while working from an aluminum extension ladder with damaged and dented rungs.  Observed on or about 07/12/2012. 29 CFR 1926.20(b)(2):     Programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    a)  10 N. Maple Ave., Marlton:  Employees were exposed to fall hazards while installing trusses for, and framing a steep sloped (8 on 12 pitch) roof with no fall protection.  Additional hazards noted included a ladder with damaged rungs and an extension cord with damaged outer coating and exposed conductors.  The employer had not initiated a program for a competent person to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition.  Observed on or about 07/12/2012.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
