29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a. On or about July 19, 2012, three employees standing on a garage roof trying to cut down a tree were not protected from falling 11 feet to the ground by the use of a guardrail system, safety net system or a personal fall arrest system.     Abatement Certification and Documentation are not required

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On or about July 19, 2012,  the employee using a saw to cut a tree was not provided with safety glasses, exposing the employee to eye injuries.    Abatement Certification and Documentation are required
