29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:          (a) Jobsite:  On or about July 19, 2012, two employees were working from a pump jack scaffold and the two aluminum pics were not overlapped over a support.  The fall hazard to the employees was approxiamtely 20 feet to the ground.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:        (a) Jobsite:  On or about July 19, 2012, two employees were working from a pump jack scaffold and the guardrail was missing an approximately 11 foot section of midrail.  The fall hazard to the employees was approximately 20 feet to the ground.

The fall hazard to the two employees was approxiamtely 20 feet to the ground.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:          (a) Jobsite, Outside:  On or about July 19, 2012, the company was using an electrical power strip that was not approved for outdoor use.  The power strip was being used to supply power to an air compressor and a Makita cut off saw.
