29 CFR 1910.134(e)(1):     The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    (a) Location - Establishment:  On or about July 19, 2012 where the employer provided tight-fitting air purifying elastomeric respirators for employee comfort and sometimes required use for work with hazardous chemicals, the employer did not ensure employees were medically able to wear the negative pressure respirators.

29 CFR 1910.1450(e)(1):     Where hazardous chemicals were used in the workplace, the employer did not develop and carry out the provisions of a written Chemical Hygiene Plan which was capable of protecting employees from health hazards associated with hazardous chemicals:    (a) LOCATION- Establishment:  On or about July 19, 2012, the employer did not ensure the protective measure in section E.1.e (blast shield and/or glass wrap) of the Chemical Hygiene Plan was used to protect employee for potential explosion hazards.     b)  Location - Establishment:  On or about July 19, 2012,the employer did not ensure the protective measure in section E.1.l (planning) of the Chemical Hygiene Plan was used to protect employees from dangers of hazardous chemcials.
