29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a) Construction project at the Redhorse Combat Communication worksite, Anderson Air Force Base, Yigo, Guam. On or about July 24, 2012 and at times prior thereto, the employer failed to have defective portable ladders either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. The ladders had missing rungs, and other defective wood components that exposed workers to serious injuries from falls.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59) (a) Redhorse Combat Commmunication worksite at Anderson Air Force Base, Yigo, Guam. On or about July 24, 2012, the employer failed to ensure that its employees were protected by training them on the hazards associated with the use and mixing of Hanson Permanente, JIS R 5210 cement. Employees were not trained on personal protective equipment, and health and safety hazards associated with the cement.
