29 CFR 1926.102(a)(1):     Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    On or about 30 July 2012, and at times prior thereto, two employees were utlizing a pneumatic nail gun to nail in shingles on the roof of a two-story house under construction without the use of safety glasses, exposing the employees to the hazard of being struck by flying objects.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure workers have and utilize safety glasses while operating nail guns or other tools that can cause flying materials.

29 CFR 1926.501(b)(11):     "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    On or about 30 July 2012, and at times prior thereto, three employees were working on elevated surfaces at heights above 6 feet high, and on steep roofs with a pitch of 6 to 12, without the use of fall protection, exposing the employees to fall hazards of approximately 10.5 feet or more to the ground below.    A) At the residence under construction located at 1915 Autumn Run Lane, two employees were performing roofing activities on the second story roof with a pitch of 6 to 12, without the use of fall protection. The first floor roof eave to ground measured at approximately 127 inches or ~10.5 feet.    B) At the residence under construction located at 1921 Autumn Run Lane, one employee was performing roofing activities on a roof with a pitch of 6 to 12, without the use of fall protection. The roof eave to ground measured at approximately 125 inches or ~10.5 feet.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure workers are protected from falling while working on steep sloped roofs that are above 6 feet in height.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about 30 July 2012, and at times prior thereto, an employee was accessing the roof of a residence under construction to perform roofing activities using an aluminum 20-foot extension ladder that was not extended at least 3 feet above the upper landing, exposing the employee to a fall hazard of approximately 10.5 feet.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure ladders are extended at least 3 feet above the upper landing.
