29 CFR 1904.39(a): An oral report of an employment accident resulting in a fatality or the inpatient hospitalization of three or more employees was not made within eight (8) hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration or to the OSHA toll-free central telephone number (1-800-321-OSHA):    The employer did not report the death of an employee from an employment accident that occurred on July 15, 2012.   The employer was informed of the employee's death on July 17, 2012 but did not make an oral report of the fatality to OSHA.  OSHA was informed of the fatality on July 26, 2012 by the North Bay Village Police Department.
