29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a)  Roof Check, Inc. at 4545 South Monoco, Unit 458, Denver, CO 80237:  On or before July 28, 2012, the employer allowed employees to use a Gradall 534D rough terrain forklift to support a scaffold platform consisting of a wood pallet, slid onto the forks, to elevate an employee cutting cement shingles and shingling a roof.  Condition exposed this employee to fall hazards of 11'6", and struck-by hazards from falling cement shingles.

29 CFR 1926.451(c)(2)(v):  Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork, and the fork-lift was moved horizontally while the platform was occupied:    a)  Roof Check, Inc. at 4545 South Monoco, Unit 458, Denver, CO 80237:  On or before July 28, 2012, the employer allowed employees to use a Gradall 534D rough terrain forklift to support a scaffold platform consisting of a wood pallet, slid onto the forks, to elevate an employee cutting cement shingles and shingling a roof.  The pallet was not tied to the mast.   Condition exposed this employee to fall hazards of 11'6", and struck-by hazards from falling cement shingles.

29 CFR 1926.451(g)(1):  An employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  Roof Check, Inc. at 4545 South Monoco, Unit 458, Denver, CO 80237:  On or before July 28, 2012, the employer allowed employees to use a Gradall 534D rough terrain forklift to support a scaffold platform consisting of a wood pallet, slid onto the forks, to elevate an employee cutting cement shingles and shingling a roof.  The employee was not tied off.   Condition exposed this employee to fall hazards of 11'6".
