29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    a) Jobsite: 1305 S Phillip St., Philadelphia, PA. - On or about 7/23/12, no conventional or alternative means of fall protection was used to protect employees from falling off the unprotected open sides and edges of a residential construction building. The employees were performing framing operations approximately 22 feet above ground level.    LPS CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 314235243, CITATION# 1, ITEM# 1A, AND WAS AFFIRMED AS A FINAL ORDER ON 10/14/11, WITH RESPECT TO A WORKPLACE LOCATED AT 1020-1026 SOUTH 5TH STREET, PHILADELPHIA, PA.
