29 CFR 1926.451(e)(1):   When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:    At the site, on August 3, 2012, an employee was accessing the second level of a three-level tubular welded scaffold, at a height of approximately 14 feet, by climbing up from the welded frame below.

29 CFR 1926.452(a)(4):   Diagonal bracing in both directions were not installed across the entire outside face of all double-and single-pole scaffolds:    At the site, on August 3, 2012, on the north wall of the building, at the east end, second level, employees were exposed to a fall of approximately 14 feet, where they were applying stucco from a section of tubular welded scaffolding which did not have crossbracing installed.
