29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.    a) Domestic Uniform Rentals/Philadelphia: The employer did not ensure that the waste water treatment and chemical mixing areas of the facility were kept orderly while traversing through the passageway of the production areas. This was observed on August 2, 2012.

29 CFR 1910.22(a)(2):     The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.    a) Domestic Uniform Rentals/Philadelphia -  The employer did not routinely maintain  a dry floor floor as practiacal outside of the dikes containing the large totes/ containers of industrial soap and hydrogen sulfide. This wet surface now exposes the employees to injuries from slips and falls.  This was observed on August 2, 2012.

29 CFR 1910.134(c)(2)(i):     Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:          a) Domestic Uniform Rentals/Philadelphia - The employer did not provide his employees who were given elastomeric, half-face respirators for voluntary use, appendix D of the respiratory protection standard. This was observed on August 2, 2012.

29 CFR 1910.134(c)(2)(ii):     The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    a. Domestic Uniform Rentals/Philadelphia - The employer provided elastomeric, air-purifying, negative pressure respirators for their employee's voluntary use comfort while working with and adjacent to chemicals in the facility. This was observed on August 2, 2012.

29 CFR 1910.151(c):     Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    a) Domestic Uniform Rentals/Philadelphia - The employer provided multiple eye wash stations [e.g. pre-treatment, boiler and the waste water treatment areas] throughout the facility that were inoperable. Multiple corrosive products  [e.g. Sodium Hydroxide, Sulfuric Acid, etc...] are used by staff which can lead to dermal and eye damage. This was observed on August 2, 2012.

29 CFR 1910.151(b):     In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.    a) Domesic Uniforms Rentals/Philadelphia - The employer provided multiple first aid kits throughout the facility that were observed as empty. No supplies were in the two kits inspected. This was observed on August2, 2012.
