29 CFR 1926.1430(d):  The employer did not train each competent person and each qualified person regarding the requirements of this subpart applicable to their respective roles:    On or about July 5, 2012, the employer did not ensure that competent persons and qualified persons involved in the placement of bridge girders over Lake Butte des Morts, Bridge B-70-266, were trained on the requirements associated with multiple crane lifts.  Employees were thereby exposed to the hazards associated with crane and load failures.

29 CFR 1926.1432(b)(1):  The multiple crane lift was not directed by a person who met the criteria for both a competent and qualified person, or by a competent person who is assisted by one or more qualified persons (lift director):        On or about July 5, 2012, a multiple crane lift of bridge girders over Lake Butte des Morts, Bridge B-70-266, was not directed by a person who was both competent and qualified to act as the lift director.  Employees were thereby exposed to the hazards associated with crane and load failures.
