This condition exposed the employees to a fall hazard of approximately 12 feet to the ground.

This condition exposed the employees to serious burns and damage to the eyes and body.      1. Eyewash machine on mud pit was inoperable.  2. Shower on shaker pit had no water.

29 CFR 1910.304(g) (5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    On or about August 3, 2012, the grounding rods at several locations on Bison Rig #62 were not properly installed; the ground rods were not inserted into the earth by NEC standards. This condition exposed the employees to electrical hazards.  1.	Rig Managers trailer  2.	Shaker Pit
