29 CFR 1910.303(b)(8)(iii): Electric equipment provided with ventilating openings was not installed so that walls or other obstructions do not prevent the free circulation of air through the equipment:  On or about July 30, 2014, and at times prior thereto, the 3-phase 208/480V Cutler-Hammer Transformer Cat No. V48M28T75 located inside the generator room was not installed so that wall would not prevent circulation of air through the ventilating openings of the transformers. National Electrical Code (NEC) Article 450.9) requires proper clearances be clearly marked on the transformer so that proper ventilation be provided to prevent them from overheating and exposing employees to fire hazards.
