29 CFR 1926.251(a)(2)(i):  Employer did not ensure rigging equipment had permanently affixed and legible identification markings as prescribed by the manufacturer that indicated the recommended safe working load:    a) On 07-26-12, employer did not ensure the four (4) American Hoist and Derrick Company pulleys used as part of rigging had permanently affixed and/or legible identification markings of the safe working load prior to hoisting and setting more than fifteen (15) concrete tilt-up panels.      ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1926.251(a)(2)(iii):     Employer did not ensure the rigging equipment in use was affixed with legible identification markings, required by paragraph (a)(2)(i) of this section.    a) On 07-26-12, an employee was using rigging to hoist concrete tilt-up panels, which included four (4) American Hoist and Derrick Company pulleys, of which each did not have permanently affixed and/or legible identification markings of the safe working load.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
